DOWNLOAD 2024 – 2025 Moose Factory Ministik School Plan
Student Registration & Information Form
A paper copy of our MFIDSAB Student Registration & Information Form is required for each student enrolled at Moose Factory Ministik School for the 2024-2025 School Year.
Parents of New Students are also required to provide copies of the following documents to complete their child’s registration: Proof of Birth, Immunization Record, Proof of Status (if applicable), and most recent Report Card, if previously enrolled in school. https://mfidsab.ca/caregiver-communication-new/new-student-registration/
In School Learning & directed Remote At Home Learning
Although all Ministik School Students will participate In School Learning, should we receive direction, Ministik School is prepared to pivot to Remote At Home Learning for all students. Unfortunately, we do not have optional Remote At Home Learning for parents to choose at this time.
Parents and caregivers have a unique responsibility to ensure their children are active participants in their learning; this is true during In School Learning and directed Remote At Home Learning. Parents and caregivers must engage in regular communication with the teacher regarding details on the learning plan and expectations specific to each class.
Self-Screening
All members of our school community, (students, staff and school visitors) who are feeling ill or have tested positive for COVID-19 should self-screen before entering Moose Factory Ministik School. The use of this online self-assessment tool is recommended:
https://covid-19.ontario.ca/school-screening/
Ill individuals should not attend school.
When an individual (student or staff member) develops symptoms (and/or self-screen indicates “stay home”), they must isolate, seek testing, and appropriate medical attention. The Moose Factory Health Center 705-658-4220 can provide guidance, as needed.
→ In case of emergency, please visit the Weeneebayko General Hospital Emergency Department. If you need an ambulance, call 705-658-1111.
